Graduate School Unacceptable Behavior Incident Report Form
It is the policy of the University of Maryland to maintain the campus as a place of study and work for students, faculty and staff in which all parties are expected to uphold the values of the University by conducting themselves in accordance with the University policies and procedures. Such an environment must be free of intimidation, fear, coercion, reprisal, harassment, bullying or other unacceptable behaviors.
If you have been impacted by and/or have witnessed an unacceptable behavior incident involving a graduate student, you can file a report/complaint by completing the online form below. Required fields are indicated with an *. We ask undergraduate students to please file reports through these channels.

Persons reporting an incident may self-identify or remain anonymous. If you self-identify, you will receive a response. Anonymous reporting, however, will limit the University’s ability to respond and support. The information will be treated confidentially.
